The preoperative cardiovascular risk management accounts for patient related risk factors, the circumstances leading to the surgical procedure, and the risk of the operation. While urgent operations should not be delayed for cardiac testing, an elective surgical intervention should be postponed in unstable cardiac conditions. In stable cardiac situations, prophylactic coronary interventions to reduce the risk of perioperative complications are rarely indicated. Therefore, in most cases, the planned operation can be performed without previous cardiac stress testing or coronary angiography. Preoperative imaging stress testing is recommended for patients with poor functional capacities that are at high cardiovascular risk prior to a high-risk operation. According to the literature, preoperative prophylactic administration of beta-blockers and aspirin is controversial. Preoperative discontinuation of dual anti-platelet therapy within six months following drug-eluting stent implantation is not recommended. [ABSTRACT FROM AUTHOR]
La stratification du risque cardiovasculaire préopératoire comprend les facteurs de risque lies au patient, à son général et est fonction du type de Chirurgie. Tandis qu'une opération urgente ne peut pas être retardée par un examen cardiologique, une intervention planifiée doit être reportée au cas d'une situation cardiaque instable. Chez les patients stables, une intervention coronarienne prophylactique pour réduire les complications peropératoires est rarement indiquée. Dans la majorité des cas, on peut procéder à l'opération sans recourir à une épreuve d'effort ou une coronarographie. Pour les patients ayant une capacité fonctionnelle réduite, un risque cardiovasculaire élevé et une opération jugée à haut risque, il est recommandé d'effectuer une imagerie cardiaque de stress préopératoire. En raison de données actuelles, l'initiation prophylactique d'un traitement par aspirine et bétabloquant reste controversée. En revanche, l'arrêt préopératoire d'une double anti agrégation plaquettaire existante dans les six mois suivant l'implantation d'un sent à élution médicamenteuse n'est pas recommandée. Bei der präoperativen kardialen Risikostratifizierung werden Patientenbezogene Risikofaktoren, die Umstände, die zur Operation führen und das Operationsrisiko berücksichtigt. Während eine Notfalloperation ohne präoperative Abklärungen durchgeführt werden soll, haben instabile kardiale Situationen Behandlungspriorität gegenüber einer elektiven Operation. Bei stabilen Patienten sind prophylaktische Koronareingriffe zur Verhinderung perioperativer Komplikationen sehr selten indiziert. Ein präoperativer kardialer Stresstest oder eine Koronarangiografie wird daher nur in wenigen Fällen veranlasst. Die Durchführung eines Stresstests wird nur für Patienten mit schlechter Leistungsfähigkeit und hohem kardiovaskulärem Risiko vor einer Hochrisiko-Operation explizit empfohlen. Der Nutzen einer perioperativen prophylaktischen Gabe von Betablockern und Aspirin ist gemäss neueren Daten umstritten. Die präoperative Unterbrechung einer dualen Plättchenaggregationshemmung innert sechs Monaten nach Implantation beschichteter Koronarstents sollte dagegen nach Möglichkeit vermieden werden.
