Introduction: Obsessive-compulsive disorder (OCD) is often comorbid with other mental disorders. Given the important clinical implications of the association between OCD and psychotic disorders, the complex nature of this comorbidity has received increasing attention. Schizo-obsessive disorder has been proposed as a diagnosis for patients who exhibit symptoms of both OCD and schizophrenia. Objectives: The aim of the present work is to review the most recent knowledge on the comorbidity between OCD and psychotic disorders such as schizophrenia, starting from the case of a patient diagnosed with OCD and psychotic features. Methods: The case of a patient with severe obsessive-compulsive symptoms since his late teens and who has experienced two psychotic states during the course of OCD so far is presented and discussed. Additionally, the authors collected the available literature through a search on Pubmed/Medline databases using the key terms "obsessive-compulsive disorder", "psychosis" and "schizoobsessive disorder". Results: Several theories have recently been proposed to explain the co-occurrence of OCD and psychosis, including their temporal interrelationship. There is empirical evidence to suggest that a prior diagnosis of OCD constitutes a risk factor for later development of schizophrenia spectrum disorders. Despite the high incidence of psychosis in OCD, most cases cannot be diagnosed as schizophrenia according to current diagnostic systems. Conclusions: The presented case is believed to fit in the schizoobsessive spectrum due to the evidence of symptoms of OCD and schizophrenia. Further studies are needed to a better understanding of the factors involved in the association of these two conditions. [ABSTRACT FROM AUTHOR]
