A man in his late 50's experienced a lack of efficacy during his anticoagulation treatment with warfarin for mitral valve stenosis. After discontinuing warfarin, he developed rebound hypercoagulability. The man had end-stage renal disease and underwent multiple surgeries in the past. Despite adhering to therapeutic anticoagulation with warfarin for 11 months, his condition worsened, and a newly developed left atrial thrombus was discovered.
