A 37-year-old man with ischaemic cardiomyopathy experienced a lack of efficacy during treatment with aspirin and heparin for thrombus prevention. The man had undergone total artificial heart implantation nine days prior to presenting with symptoms such as right gaze preference, left facial droop, and global aphasia. Acute imaging revealed a thrombus in the left middle cerebral artery territory, indicating that the aspirin and heparin therapies were not effective. The patient was hospitalized and underwent mechanical thrombectomy, which resulted in the resolution of the mismatch in the affected territory.
