The article describes a framework for optimizing medical practice through health sciences research. The authors describe five principles which they believed could reduce frustrations of policy makers, researchers, clinicians and the public regarding improvement in patient outcomes. The principles include the need of patients and populations determine the research agenda, the research agenda addresses contextual and implementation issues and research method which determine the research agenda.
